Historic Preservation Program Application Forms

To nominate or to request approval to alter landmarks in Seattle, or to apply for approval to alter properties within established historic districts, please use the appropriate form below, which may be printed in PDF format. Modifications to individually designated landmarks and the properties within the Columbia City and Harvard-Belmont historic districts require Certificates of Approval from the Landmark Preservation Board. Modifications to properties within the Ballard Avenue and Pike Place Market districts and within the Pioneer Square and International Special Review Districts require special applications for Certificates of Approval from their respective governing boards.
