About the use of these forms.
These forms are available to print out and present to the court at your court hearing.
Please two hole punch the forms at the top for inclusion in the case folder per SMCLR 8.4.e.
(All papers filed with the court shall comply with GR14 and shall have two holes punched at the top (8 1/2 inch edge). These holes shall be 2 3/4 inches apart centered approximately 1/2 inch from the top edge of the paper. Papers which do not conform with this rule may be rejected by court clerks.)
Altered forms will not be accepted by the court.
SMC is not yet able to accept these forms electronically via email or otherwise.
