Street Maintenance Request Form

a.k.a Report A Pothole!

Our goal is to repair potholes within 48 hours (two business days) of receiving a report. Currently our success rate is 94%.

Use this form to report non-emergency problems such as pot holes and other minor street repairs, malfunctioning traffic signals, and damaged street signs.
